Photo, Video and Liability Waiver Release Form

One release form per adult, please.

Every adult (18 and older) in your party must complete their own Photo & Video Release—each visitor signs individually, including parents or legal guardians. If you are a legal guardian or parent attending with minor children, only one of the parents or legal guardians needs to include their minor children in the form.

Terms of Release

I, the undersigned, hereby certify that I am an adult visitor (18 years of age or older) and/or the legal parent or court-appointed guardian of the minor child(ren) listed below.

Media Permission:

I grant Asha's Farm Sanctuary, Inc. ("the Sanctuary") and its authorized representatives the irrevocable right and permission to photograph, film, record, and use images and/or video footage of me and the minor child(ren) listed below during any visit to the Sanctuary.

Usage Rights:

I authorize the Sanctuary to use, reproduce, publish, display, and distribute such photographs and video recordings—in whole or in part—in any media format, including but not limited to: the Sanctuary's website, social media platforms, newsletters, fundraising materials, educational resources, and print publications.

Compensation & Inspection:

I understand that neither I nor the minor child(ren) will receive monetary compensation for the use of these images or recordings. I waive any right to inspect or approve the finished product and release the Sanctuary from any and all claims arising from such use.

Assumption of Risk & Liability Waiver:

By signing below, I acknowledge that visiting a farm sanctuary involves inherent risks, including but not limited to unpredictable animal behavior, uneven terrain, and outdoor conditions. I agree to assume all risks and release the Sanctuary, its directors, employees, and volunteers from any liability for personal injury, illness, or property damage occurring during any visit to the Sanctuary.

This release is binding upon me, the minor child(ren), our heirs, legal representatives, and assigns.
